POA

(Proof of Address)


Water Bill
A receipt of payment for water services provided by the Ghana Water Company Limited(GWCL).

-Submit a recent bill payment receipt issued within the past 6 months


-Ensure this receipt is in your own name and has a valid address


-Ensure your name and address information are visible on the document


1. Visit the nearest Ghana Water Company Limited(GWCL)  office or utility bill vending point in your area.


2. Provide your account number/property address and make bill payment.


3. Request for a bill receipt after payment


OR


1. Request for a digital copy via email if you have registered for online services


Electricity Bill
An electricity bill is a document issued by the electricity provider(ECG / NEDCo) showing the cost of electricity consumed over a billing period.

-Submit a recent bill payment receipt issued within the past 6 months


-Ensure this receipt is in your own name and has a valid address


--Ensure your name and address information are visible on the document


1. Visit the nearest Electricity Company of Ghana(ECG) Office or any utility bill vending point in your area.

2. Provide your meter number/account number/prepaid card.

3. Request for a bill receipt after payment of your bill.


OR- For ECG App Users:


1. Log in with your registered phone number and password.

2. Navigate to the "Bills" or "Transactions" section on the App to view your current/past bills.

3. To make a bill payment, select the desired bill from the list, choose the preferred payment method and follow the prompt to complete payment.

4. The ECG App will generate a bill for you once the payment is completed. 

5. You can share a screenshot of this bill on the Fido App to improve your Fido Score.


Tenancy Agreement

A contract between a landlord and a tenant outlining the terms and conditions of renting an apartment.

-Submit an agreement that has your name as tenant and a valid address clearly stated.


-The agreement must be signed by both landlord and tenant.


-Ensure the document contains all agreed terms including your rent amount and period of tenancy


1. Request for a signed copy of your tenancy agreement from the landlord/landlady of your rented apartment.


2. You can also ask your landlord or a Legal Practitioner to draft or review your agreement.


Rent Card
A document issued/endorsed by Rent Control Department to record and track rent payments made by a tenant to a landlord

-Ensure your name is stated as a tenant on the rent card


-Ensure all required information such as the rent amount, commencement date and valid address are clearly stated on the card


1. Visit your Local Housing Authority or District Assembly

2. Submit an application and provide valid identification  e.g. Ghana Card.

3. Once the application is approved, the rent card will be issued to you.


POE

(Proof of Employment)


SSNIT Statement
A document that provides a summary of an individual's contributions to the national social security scheme.

-Submit all pages of the document as a PDF.


-The statement should be issued within the past 6 months.


-The document must have your name.


1. Visit the nearest SSNIT office with your SSNIT Number and valid ID.

2. Approach the customer service desk and provide all necessary details required.

3. Your information will be verified and printed out for you.


OR For Online Users


1. Access the SSNIT Online Portal.

2. Navigate to the "Member Portal" or "Self -Service Portal".

3. Log in with the required credentials or Sign up if you are a new user.

4. Select "Request Statement" or "Statement of Account" and enter required credentials.

5. Enter required credentials and submit statement request.

6. Your data will be reviewed and verified then you will see the option to view/download your statement.


Payslip
A document issued by an employer to an employee, detailing the employee's earnings and deductions for a specific pay period.

-Submit a recent payslip issued within the past 6 months.


-Your full name, employer's name and salary amount must be clearly stated on the document.


1. This can be requested for from your hiring manager or the Human Resource Department of your company if you do not already have one.


For Civil Servants:

 

1. Visit the Controller and Accountant General's Department(CAGD) official Electronic Salary Payment Voucher portal(E-SPV) to access your E-Payslip


2. Log in with your Employee Number and Password or Click on Register/Sign up if you are a new User.


3. Navigate to the Payslip section


4.Select and download payslip. 


5.Save document as PDF or print directly from the portal.


Appointment Letter
An appointment letter is issued by an employer to an employee, confirming the employee's working status within the organization

-Submit all pages of the document ensuring your employer's name, position and salary amount are clearly stated.



-The document must necessarily have your employer's name and signature.

-Submit a recent letter issued within the past 12 months.


 This can be requested for from your hiring manager or the Human Resource Department of your company if you do not already have one.
Introductory Letter
A formal letter by an employer or an organization to introduce an individual to another organization or institution.

-Submit a recent  introductory letter, issued within the past 6 months.


-This letter must be from your employer, introducing you to Fido.


-Your full name, employer's name and position must be clearly stated in the letter


-The document must necessarily have your employer's name and signature.


This can be requested for from your hiring manager or the Human Resource Department of your company if you do not already have one.



Promotion Letter
An official communication issued by a company to an employee, informing them that they have been promoted to a higher position within the organization

-Submit all pages of the document ensuring your employer's name, position and salary amount are clearly stated.


-The document must necessarily have your employer's name and signature.

-Submit a recent letter issued within the past 12 months.


This can be requested for from your hiring manager or the Human Resource Department of your company if you do not already have one.
Salary Increment Letter
A formal letter to communicate an increase in an employee's compensation, often awarded annually or based on performance, experience

-Submit all pages of the document ensuring your employer's name, position and salary amount are clearly stated.


-The document must necessarily have your employer's name and signature.

-Submit a recent letter issued within the past 12 months.


This can be requested for from your hiring manager or the Human Resource Department of your company if you do not already have one.
Business Form A
A business name registration document for Sole Proprietorships issued by the Registrar General's Department(RGD)

-Ensure your name appears as Proprietor/Proprietress on the document


-Ensure to submit all pages of the document as a PDF.


1. Visit the Registar General's Departmnent office or access the RGD Online Portal 

2. Request for a Form A and complete the necessary forms and submit them along with the required documentation.

3. Pay the applicable fees and submit the completed forms.

4. Once your submitted document is reviewed and approved, you can collect your business document.


OR  access the RGD Online Portal 


1. Log in with your account credentials or create a new one to access the portal.

2. Select the option to register a Partnership and fill form online.

3. Upload supporting documents as requested on the pages(Identification documents).

4. Pay the required registration fee after reviewing and submitting the form electronically.

5. Once application is completed, you will receive a notification. Then download the document from the portal or receive it via email.


Business Form 3
A business name registration document for Partnerships issued by the Registrar General's Department(RGD)

-Your name should be included under "Company Officer Details" on the document


-Ensure to submit all pages of the document as a PDF.


1. Visit the Registar General's Departmnent office 

2. Request for a Form A and complete the necessary forms and submit them along with the required documentation.

3. Pay the applicable fees and submit the completed forms.

4. Once your submitted document is reviewed and approved, you can collect your business document.


OR  access the RGD Online Portal 


1. Log in with your account credentials or create a new one to access the portal.

2. Select the option to register a Partnership and fill form online.

3. Upload supporting documents as requested on the pages(Identification documents).

4. Pay the required registration fee after reviewing and submitting the form electronically.

5. Once application is completed, you will receive a notification. Then download the document from the portal or receive it via email.


Financial Information
MTN Statement
Statement of Mobile Money transactions on a wallet number issued by MTN to the owner of a registered mobile number.


-Ensure the duration between the first and last transaction is not less than 60 days


-Ensure the document is recent and/or issued not more than a month ago.


You can follow this USSD procedure to request for a statement from MTN:


1. Dail *170#

2. Select option 6 - My wallet

3. Select option 5 - Statements

4. Select option 1 - Statement Request

5. Enter your MM PIN

6. Enter the email you wish to receive the statement on

7. Confirm email address

8. Choose duration 


You will receive a pop up message to confirm a successful MOMO Statement request.


OR follow these steps on the MTN APP to receive your statement


1. Login to your account with your Phone Number and PIN.

2. On the Homepage, tap on "MoMo" at the bottom navigation bar.

3. On the MoMo page, tap on the "Statement" icon and enter your MOMO PIN and enter the OTP code sent by MTN.

4. On the MoMo History page, tap on the menu bar and select "Email my statement to me"

5. Enter the email address you wish to receive the statement on and select the required duration and select NEXT.


You can check your email later for the statement from MTN after you see a pop up page saying "Request Sent".


Bank Statement
Statement of Bank transactions issued by Bank to the account owner

-Ensure the document is recent and/or issued not more than a month ago.

-Ensure the duration between the first and last transaction is not less than 60 days.

-Ensure the submitted statement is not password protected.


1. Log into your bank's Mobile App with your credentials or Sign Up to create a new account.

2. Navigate to "Statements" or "Account Statements" in the main menu or dashboard of the interface

3. In case you have multiple accounts with your bank, you can select the preferred account you want a statement from

4. Enter/Select the required duration

5. Download the statement in a readable format(preferably PDF) OR Request to receive it as an email.


Email Verification
Email
Customer's email address used on Google Play Store or Appstore linked to the Fido App to improve customer's Fido Score
-Ensure the email provided is active on Play Store or AppStore.
